According to McClelland, these needs are found in every individual, and they determine the manner in which an individual interacts with others. Under achievement, an individual sets to realize the goals that he or she has set up to achieve. There is always a strong need of accomplishment, which must be followed by feedbacks. The second need is the need of power. Under this principle, an individual has the desire to have authority, and to be a decision maker. This also involves the desire for a person to increase his or her prestige and social status. The third need is the need for affiliation (Karabenick and Urdan, 2010). Under this need, an individual needs friends, and therefore there is a desire to interact and socialize with his or her peers. Another theory of motivation is the Herzberg’s Motivator-Hygiene Model. Under this theory, Herzberg denotes that there are factors that can cause job satisfaction, and there are also other factors that can cause job dissatisfaction (Lange, 2012). On this basis, when an employer is developing policies in regard to motivating his or her employees, they need to know factors that will motivate them, and factors that won’t motivate them. Herzberg further identifies the concepts of motivators, and hygiene factors. Motivators are factors that provide positive job satisfaction, while hygiene factors always provide negative job satisfaction (Lange, 2012). Reflection on growth of competency in Clinical Skill For this essay I am going to reflect upon a clinical skill I have become competent in during my training. I will use a reflective model to discuss how I undertook the skill. I will also discuss the rational and research behind the skill. The model of reflection I have chosen is Gibbs (Siviter, 2004). The Gibbs model of reflection allows the skill to be critically analysed. This is achieved by using the following sub headings; description, feelings, evaluation, analysis, conclusion and an action plan (Siviter, 2004). The skill that I will reflect on in this essay is the administration of an intramuscular Injection (IM). An IM is an injection deep into a muscle (Dougherty Lister, 2008). This route is often chosen for its quick absorption rate and often medication cannot be given via other routes. The reason I have chosen to reflect on this skill is because I have had many opportunities to perform this skill, and at my current practice placement this is the most commonly used method of drug administration. I have undertook many IMs at this placement but I am going to reflect on the first one I undertook which was the administration of Hydroxocobalamin commonly known as vitamin B12 (BNF, 2007) Description During a morning clinic with the practice nurse, I was asked if I would like to administer an IM on the next patient, which was a 26 year old lady who has been suffering from crohns disease which can cause B12 deficiency due to lack of vitamin and mineral absorption (NACC, 2007). I agreed and she briefly went through with me how to do an IM as it had been a while since I had last done one. I called the patient in and asked her to sit down. The patient had come in for her first injection of B12. I chatted to the patient asking her how she was and if she had any concerns. I then gained consent asking her if it was ok for me as a student to administer it under the supervision of the practice nurse. The patient responded with you have got to learn I then prepared the equipment which included two needles, a sharps box, a piece of gauze and the medication. I checked the prescription with the practice nurse, and then checked the ampoule against the prescription. I then drew up the medicatio n with one needle disposing of it in the sharps box and attached the other needle. I then proceeded to administer the medication, after completing the procedure I disposed of the needle in the sharps box and documented it in the patients notes. After the patient had left the nurse explained to me I had done it all correct except I had gone in too far so if the needle broke it would be hard to get it out and that I didnt aspirate to check if I had gone into a vein. Thoughts and feelings After I was asked if I wanted to do the IM I felt very anxious as it had been more than 6 months since the last time I had administered one. But she explained the procedure to me which relieved some of my anxiety. When I first met the patient I was feeling allot more nervous as the patient was roughly my age and I havent had much experience of caring for the younger person. After the procedure when I was told I was wrong for not aspirating I felt annoyed as I was sure I had read that aspirating was no longer necessary. Evaluation Overall I feel that the clinical skill went well as a whole. I followed the instructions from my mentor and what the research has suggested other than feeling a little anxious I performed the skill confidently and correctly. What I feel was bad about the experience is with my communication, which reflecting on I believe was lacking. I communicated with the patient prior to the skill and after the skill, but during I felt I almost forgot there was a patient on the end of the needle. I was so focused on getting the skill right and not causing any pain I didnt talk to the patient throughout the whole thing. Another point that I feel was bad is, I forgot to wear an apron. My mentor never mentioned anything about this although I do feel I should have worn one as its an aseptic technique and its part of the (DOH, 2006) guidelines. Analysis The reason why an IM injection was chosen is because B12 can only be administered via IM (BNF, 2007). I gained informed consent off the patient as this is part the NMC guidelines. (NMC, 2008) As patients have the right to decline treatment. After gaining consent, I then checked the medication against the patients chart to ascertain the following: Drug, Dose, date, route, the validity of the prescription and the doctors signature. This is done to make sure the patient receives the correct drug and dose (NMC, 2008) I then washed my hands using Ayliffes six step technique to reduce the risk of infection and put gloves on as part of DOH 2007 Guidelines . The site that I chose was the mid deltoid site. Hunt (2008) Suggests that this is the best site to use as its easy to access whether the patient is sitting, standing or lying down, it also has the advantage of being away from major nerves and blood vessels. Although Roger (2000) states that only 2ml at most can be injected into the delto id. I was able to proceed with this site as B12 comes in a 1ml dose (BNF, 2007). I asked her if she would prefer to sit or lie down, she said she rather sit, this was ok with me as I am not very tall and found this a comfortable position for me. As the patient was wearing a short sleeve top I asked her to move it up slightly instead of removing it thus allowing her to maintain her privacy and dignity. I then assessed the injection site for suitability checking for any signs of infection, oedema or lesions. This is done to promote the effectiveness of administration and reduce the risk of cross infection (Woorkman, 1999). Holding the needle at a 90 degree angle it is quickly pushed into the muscle. Workman 1999 says this ensures good muscle penetration. I inserted the needle leaving approximately 1/2cm exposed as Workman, (1999) says this makes removing it easier should it break off. At this point I decided not to aspirate as per research (DOH, 2006). After inserting the needle I all owed it to remain there for 10 seconds. As Woorkman (1999) suggest that leaving in situ for 10 seconds allows the medication to diffuse into the tissues. After 10 seconds had past I swiftly removed the needle and applied pressure according to Dougherty Lister (2008) this helps prevent the formation of a haematoma. Immediately after carrying out the skill I disposed of the needle into a rigid sharps container. To ensure health and safety is maintained and the used sharps dont present a danger to me or other staff members as stated by MRHA (2004). After the procedure I documented it within the patients notes as per NMC guidelines and to provide a point of reference if there ever was a query regarding the treatment and to prevent duplicate administration (NMC, Guide lines for records and record keeping, 2005). After the skill I discussed with my mentor that recent evidence suggest that aspirating is unnecessary. According to Workman (1999) the reason for aspirating is to confirm that the needle is in the correct position and to make sure that it has not gone into a vein. The most recent and up to date evidence, says that aspiration is only necessary if using the dorsogluteal  site to check for gluteal artery entry (Hunter, 2008). But official guidance from the World Health Organisation and the Department of Health (DOH, 2006) (WHO, 2004) suggest that this site should no longer be used, thus making aspiration unnecessary. By not aspirating it makes the procedure simpler and less chance of adverse events. Furthermore pharmaceutical companies are making less caustic preparations and in smaller volumes. I discussed this with my mentor and she agreed but stated that it is PCT policy to aspirate, and she would have to continue to follow this practice until the policy was amended. Conclusion Using the Gibbs model of reflection has allowed me to thoroughly analyse the event and allowed me to explore my feelings. I have found out despite the evidence being constantly up to date that not all practitioners knowledge is as up to date, and that trusts are equally as slow to adopt new ideas within their policies and that nurses are governed by policy more than current research. I have also learned that there is a great deal of evidence behind such what on the outside seems to be a simple technique and what I thought I was doing correctly may not always be the case. Action plan I do not doubt I will be carrying out IMs for a long time in my career. I will not be doing much differently in the future as the evidence is underpinning my practice. I will not put the needle in as far as I did on this occasion. In the future I will continue not to aspirate, unless local policy indicates otherwise. In addition I will communicate with the patient throughout the entire skill and not just at the start and end of. Whats more from this event I have realised that learning never stops and what I know now may not be relevant tomorrow.

Inferno - Contrapasso In Dante’s Inferno, Dante takes a journey with Virgil through the many levels of Hell in order to experience and see the different punishments that sinners must endure for all eternity. As Dante and Virgil descend into the bowels of Hell, it becomes clear that the suffering increases as they continue to move lower into Hell, the conical recess in the earth created when Lucifer fell from Heaven. Dante values the health of society over self. This becomes evident as the sinners against society experience suffering greater than those suffer which were only responsible for sinning against themselves. Dante uses contrapasso, the Aristotelian theory that states a soul’s form of suffering in Hell contrasts or extends their sins in their life on earth, to ensure that the sinners never forget their crimes against God. Even though some of the punishments the sinners in Hell seem arbitrary, they are fitting because contrapasso forces each sinner to re-live the most horrible aspect of their sin to ensure they never forget their crimes against God. As Dante and Virgil, Dante’s guide through Hell, approach the Gate of Hell, Dante reads the inscription above the gates: â€Å"Through me the way to the suffering city, through me the way to the eternal pain, through me the way that runs among the lost. Justice urged on my high artificer; my maker was divine authority, the highest wisdom and the primal love. Before me nothing but eternal things were made, and I endure eternally. Abandon every hope, who enter here.†Ã‚  Ã‚  Ã‚  Ã‚   (III, 48) This message accurately describes how those souls will experience contrapasso in Hell. They will never be released and will experience suffering for eternity. The first line speaks of a... ...ouls to exist. It is a fitting punishment because he wanted to rule like God. Now he does, but he rules over the souls who could not achieve the presence of God after death. Dante efficiently uses contrapasso to punish the souls that sinned in their lifetimes. All the sinners experience ultimate suffering as they act to extend or continue their sin for eternity. The suffering in Hell is ultimately unbearable, regardless of the nature of sin. The sinners have no hope of their condition becoming any better because the only change will be at the Final Judgment. Then their punishments will be perfected because they will then have bodies and a new way to experience suffering. Does ‘Better’ corporate governance cause better firm performance? Essay

A study had been generated and said that performance are affected either the implemented governance is good or bad: †¦ In summary, our results are as follows. First, we do not find significantly different firm performance between firms with â€Å"good† changes in governance and firms with â€Å"bad† changes in governance, except for isolated instances. This result holds for all the three samples (i. e. the Moderate Performance sample, the Abnormally Bad Performance sample and the Abnormally Good Performance sample). Second, both â€Å"good† and â€Å"bad† changes in governance are followed by positive as well as negative changes in industry-adjusted performance. Indeed, more than 50% of the firms with â€Å"good† governance changes exhibit negative industry-adjusted performance. Third, we find that both â€Å"good† and â€Å"bad† changes in governance lead to significant changes in performance. Therefore, if one restricts the analysis to only â€Å"good† changes in governance or only â€Å"bad† changes in governance, one would incorrectly conclude that changes in governance lead to changes in performance. Fourth, we observe that different firms vary in directions of governance changes when they experience the same direction in performance changes. A same firm also often simultaneously changes its different governance characteristics in conflicting directions (i. e. both â€Å"good† and â€Å"bad† governance changes). Last, our measure for Aggregate Governance Change confirms the first result for individual governance measure that the firms with â€Å"good† changes in governance do not have better performance than the firms with â€Å"bad† changes in governance. Our results present strong evidence against the null hypothesis that â€Å"better† governance causes better firm performance. Note that our results do not imply that governance is irrelevant. Instead, it implies that firms are endogenously optimizing their governance structure in response to observable and unobservable firm characteristics. These results are consistent with the strand of the literature that has shown each governance mechanism to be related to observable and unobservable firm characteristics. The statement proves that all decisions and change causes an effect to a business. This however means that every day, new challenge may face the company. It is normal for demands, and conflicts to happen. Hence, the company must be very careful of choosing the right people to trust and the right people for a certain job. Documentation is always a must. Never let any situation happen without any documentation. All reports and transactions must be done with papers and with the PIC’s (Person in Charge) signature to make it even more valid and powerful in meetings and agreements. Even important conversations must be recorded and documented to ensure understanding between parties. Meetings are also important and dissemination of information is a must. Memoranda should always be disseminated to all employees. Discipline has been an issue in all companies. The company must enforce a strict and fair rule and policies that the employees must do in order to organize a company. Make them come in the company on time and finish their work fast. Over time on work is not a measurement of quality service, it only show two things: its either the worker can’t handle too many work because of pressure or multi tasking or the worker can’t finish the work on time because he/she wasted it. The company should always have a room for improvement. Benchmarking or company observation is a big help to improve the company’s usual workflow and environment. People used to compare one thing to another. Make it a positive way of upgrading the company. Have a group of trusted people to assist the company to make improvements. Lastly, a company must have a goal or mission. No company must have an inspiration to look forward to. Set a series of mission, vision, the company’s commitment, core values and principles so that the workers will live by the company’s principles and for the company’s goal. There are series of ways and measurements that can be done through quality audits, and meetings. Implement good data management and all department management. Social commentary on The Kite runner

We are impacted by things that we read, see or hear on a daily basis, and whether we know it or not they change the way we perceive certain topics. Social commentary Is often used by authors and/or artists to draw attention to ineffective elements of a society. This is present in the novels â€Å"Animal Farm† by George Orwell and The Kite Runner† by Khlaed Hosselnl.The â€Å"Animal Farm† Is an allegory about a farm in which the animals revolt against the irresponsible farmer Jones after the Old Major dies and name it â€Å"Animal animals establish seven rules to live by, f which the most Important Is â€Å"All animals are Farm 47). As the years pass by. though, the dynamics of the farm change and the pigs establish themselves ds the ruling class. Orwell ridicules the way Russia tried to rebuild itself after the Bolshevick Revolution with this book. The Kite Runner† is a novel written by Khaled Hosselnl, In which he talks about the drastic difference betwee n lite In Afghanistan before and after the Taliban take over through the point of view of an Afghan boy named Amir. The main themes in the book are love, forgiveness, betrayal, edemption, and ethnic pressure In Afghanistan, but he also focuses In abundance on the Taliban by using the characters' life to show how much damage the Taliban cause to the country and population. The Taliban come Into power after Russian troops invade Afghanistan, and they make the citizens believe that they are the salvation of the country.Both novels deal with a group of people/animals that start a revolution against the ones in control, then gain power and impose even more severe rules which they corrupt, and end up forming a government more oppressive than the one they revolted against. nce the groups take over they give themselves higher authority than the rest of the population/animals, for example when Napoleon takes over he Is called â€Å"the emperor† and the rest of the animals are called à ¢â‚¬Å"comrade†(Anlmal Farm 47-8).The Taliban does something very similar; they demand the population to treat them with respect. Amir finds that out when he comes back to Kabul to take sohrab (given the fact that he lived In U. S. while the Taliban took over Afghanistan, he doesnt know much about them, besides what he heard in San Francisco) and he stares at the Taliban when they drive by. His triend Farid informs him that when the Taliban move past he Is supposed to look at the ground and not directly in their eyes, for the reason that they wanted people to almost bow down to them.Both dictators Impose unreasonable rules when they are in position of power. In â€Å"Animal Farm† Napoleon demonstrates that when he says that anyone who followed snowball, the leader thrown out by Napoleon, would be executed. In â€Å"The Kite Runner† there are many scenes where the Talibs execute the opposition, for example Hassan refuses to leave Baba's house and as a result he ets shot In the back of the head. Another Intense case of unreasonable punishment takes place at the stadium where one of the Taliban Generals makes a speech about the Taliban's duty.He claims that the Taliban bring Justice and carry out the shrl'a law, and they believe that if adultery is committed, the adulterers â€Å"throw stones at the windows of God's house†: although the Shri'a law does not literally say that the Taliban Imply that adulterers should be punished by being stoned to death, tnereTore Its only rlgnt tnat tney stone to aeatn a woman ana a man accused 0T dultery(Cleric 270). However, in both books the dictators corrupt the rules they impose, for example in the â€Å"Animal Farm† the animals have seven commandments they must live by but three of them get changed to suit what Napoleon does or decides. Napoleon changes the name of the farm back to Manor Farm and in the final scene Napoleon and Pilkington (a neighbori ng farmer) are playing cards, although they act friendly, each is trying to cheat by playing ace of spades .The other animals watch the scene and cannot tell the pigs from the humans. Social commentary is often used by authors and/or artists to draw attention to ineffective elements of a society. This is present in the novels â€Å"Animal Farm† by George Orwell and â€Å"The Kite Runner† by Khlaed Hosseini. The â€Å"Animal Farm† is an allegory about a farm in which the animals revolt against the irresponsible farmer Jones after the Old Major dies and name it â€Å"Animal farm†.The animals establish seven rules to live by, of which the most important is â€Å"All animals are equal†(Animal Farm 47). As the years pass by, though, the dynamics of the farm change and the pigs establish themselves as the ruling class. Orwell ridicules the way Russia tried to rebuild itself after the Bolshevick Revolution with this book. â€Å"The Kite Runner† is a novel written by Khaled Hosseini, i n which he talks about the drastic difference between life in Afghanistan before and after the Taliban take over through the point of view of an Afghan boy named Amir.The main themes in the book are love, forgiveness, betrayal, redemption, and ethnic pressure in Afghanistan, but he also focuses in abundance on the Taliban by using the characters’ life to show how much damage the Taliban cause to the country and population. The Taliban come into power after Russian troops invade Afghanistan, and they make the citizens believe that they are the salvation of the country.Both novels deal with a group of people/animals that start a revolution against the ones in control, then gain power and impose even more severe rules which they corrupt, and end up forming a government more oppressive than the one they revolted against.
